The PHMC Health Network offers the MyPHMCHealth portal for patients receiving care at our FQHCs. Through the portal, you can directly contact health care providers, view past and upcoming appointments, access your test results, and request prescription refills at your convenience. New and returning patients must schedule appointments by calling 855.887.9229 or filling out our online form.
