PHMC Medical Respite on Cedar offers post-acute medical care for people experiencing homelessness who need medical care after a hospital stay. Located in the PHMC Public Health Campus on Cedar, this program and its PHMC counterpart Serenity Court are the only homeless medical respites in the City. The benefits of medical respite programs include better health outcomes for patients, lower re-admission rates for several months, and long-term cost savings for hospitals, insurance companies and communities.

PHMC Medical Respite on Cedar provides:

  • A state-of-the-art facility with 20 beds and comfortable space for rest and recovery.
  • High-quality and compassionate medical care, including treatment for wounds.
  • Medication management, follow-up appointments, and linkage to primary care and other PHMC resources.
  • Case management to help connect patients with social services.
  • Assistance with accessing post-respite housing.
  • A minimum 5-day supply of medication upon being discharged.
  • Behavioral health resources on site, including recovery support and educational groups, help with Medication-Assisted Treatment and linkages to treatment services in the community.

Eligibility is determined after an initial phone call based on the criteria below.

A patient must be:

  • Experiencing homelessness.
  • Independent in Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL), although individuals with ADL limitations will be reviewed on a case-by-case basis.
  • An unaccompanied adult aged 18 years or older.
  • In need of post-acute recuperative care. Includes prep for colonoscopy.
  • All patients will be screened for their ability to live safely in a communal setting.

34 Trolley Line – Exit at 53rd & 54th and Baltimore Avenue and walk to Cedar Avenue.

G Bus Route – Get off at 56th and Cedar and walk two blocks to 54th Street.
