The PHMC Health Network is committed to reducing disparities in health care access. Our Federally Qualified Health Centers (FQHCs) serve a diverse population, including residents of public housing and people experiencing homelessness.
We provide care to all patients regardless of their ability to pay and accept most insurance plans. A sliding fee scale is available for uninsured and underinsured patients. Eligibility is based on family size and income. If you are interested in the sliding fee scale program, please bring valid identification and proof of income such as a pay stub, bank statement, or W2 form.
If you do have insurance, we accept a comprehensive list of medical and dental plans from CHIP, Aetna, Cigna, Geisinger and many other commercial, Medicaid and Medicare plans.
