The PHMC Care Clinic provides primary care for adults, children, and families. It also offers specialized health services to adults with unique medical needs, including HIV and Hepatitis C.
The Care Clinic is within the PHMC Health & Wellness Center. It offers referrals to PHMC Dental and PHMC Pharmacy’s on-site services. Our team includes bilingual staff and offers interpretive services. PHMC Care Clinic is a Patient-Centered Medical Home.
We accept most insurance plans. A sliding fee scale, based on income, is available for uninsured and underinsured patients. We see all patients regardless of their ability to pay.

This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n). This health center also receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
