The PHMC Health Network’s mission is to foster healthier and stronger communities in the Philadelphia region. Our six nurse-led Federally Qualified Health Centers (FQHCs) provide comprehensive, accessible and patient-centered health services to adults, children and families. We serve more than 18,000 Philadelphians each year, including residents of public housing, people experiencing homelessness, LGBTQ+ individuals, and adults with specialized health needs.
All six of our FQHCs are designated as Patient-Centered Medical Homes. Patient-Centered Medical Homes emphasize the relationship between patients and health care providers as well as collaboration across the health system to improve the quality of care and maximize positive health outcomes.
The PHMC Health Network provides a public health approach to care – one that is preventive, focuses on the connection between physical and behavioral health, and is community-centered in addressing the social determinants of health.
Beyond our FQHCs, PHMC’s nurses provide primary care services in ten shelters across the City. They also serve as a resource to street outreach teams working with people experiencing homelessness.
The PHMC Health Network also offers post-acute medical care for people experiencing homelessness through its Serenity Court and PHMC Medical Respite on Cedar programs. These facilities provide comfortable space for recovery after hospitalization and connect patients to primary care and other services.
