PHMC Health Care for the Homeless provides high-quality and trauma-informed care for people experiencing homelessness. The program coordinates with Mary Howard Health Center and the entire PHMC Health Network to provide primary and behavioral health care and comprehensive social services to meet the complex needs of individuals experiencing homelessness.
PHMC Health Care for the Homeless aims to improve the health outcomes of the individuals and families it serves through culturally competent and patient-centered care. We achieve this mission by:
- Providing care with dignity and respect to promote a goal of self-reliance.
- Developing health solutions that use evidence-based practices.
- Creating innovative technologies through strategic partnerships with City government, communities and coalitions, and advocacy and outreach teams.
