PHMC Health Connection


PHMC Health Connection, a Federally Qualified Health Center, provides a variety of services to children, teens and adults. We partner with Norris Homes and Apartments, residences that are owned and operated by the Philadelphia Housing Authority. We have a bilingual staff and interpretation services available. PHMC Health Connection is a Patient-Centered Medical Home.

Our health care services include:

  • Adult and pediatric primary care
  • Family planning
  • HIV and Hepatitis C testing, counseling and treatment
  • Immunizations and flu shots
  • Health and nutrition counseling
  • Women’s healthcare services
  • Behavioral health and trauma informed consultation services
  • Management of chronic health conditions including diabetes, hypertension, asthma, and others
  • Clinical social workers
  • Medical case management
  • Addiction medicine
  • Peer recovery services, referral navigation, health insurance assistance
  • Social services assistance (SNAP, housing, insurance, utilities, legal etc.)

PHMC Health Connection offers a pharmacy discount program. Most insurance plans are accepted and there is a sliding fee scale available for uninsured patients. All patients will be seen regardless of their ability to pay.
