Sign Up for a MyPHMCHealth Account
PHMC Health Network has a new portal for patients receiving care at its health centers. Through the MyPHMCHealth portal, patients can directly contact their providers, view past and upcoming appointments, access their test results, and request prescription refills at their own convenience.
To create an account, sign up with a provider at your next appointment or by using the New User sign-up available at myphmchealth.org. If you are unable to sign up for an account, please call 855.887.9229 for further assistance.
