PHMC Health Network Upgrading its Electronic Health Record. See Temporary Schedule.
PHMC is upgrading its electronic health record to help us provide integrated, comprehensive, high-quality care in our health centers and convenient access to the wide network of PHMC programs and services that address social determinants of health.
During this transition, we will operate on a limited patient schedule. We apologize for any inconvenience:
PHMC Care Clinic and PHMC Health Connection
- October 26 – 30: Open from 1:00 p.m. – 4:00 p.m.
- November 2 – 13: 9:00 a.m. – 4:00 p.m., open for limited appointments
Mary Howard Health Center, Rising Sun Health Center and Congreso Health Center
- November 9-13: Open from 1:00 p.m. – 4:00 p.m.
- November 16-25: 9:00 a.m. – 4:00 p.m., open for limited appointments
As a PHMC Health Network patient, you receive comprehensive care that includes primary and behavioral care, substance use treatment and social services. At intake, every individual receives a quality of life or social supports assessment that includes medical conditions, mental health needs, behavioral and social needs, education, insurance, housing and job readiness skill building, child support services and others as needed.
Our new system will connect you with PHMC programs and services based on your individual needs including outpatient and residential programs for individuals with substance use and co-occurring disorders, health promotion and chronic disease prevention, child and family social services, K-12 education, community-based and trauma informed behavioral health services to help children, youth and families, early intervention services and special education for children and adults, and workforce development including employment readiness.
Walk-ins will be accommodated as much as possible, for services including STD testing, family planning services, HIV care, COVID testing and Medically Assisted Treatment services.
